- Permanent/Provisional Registration Certificate from MCI/NMC or State Medical Council
- Date of Birth Proof (Class 10 certificate or birth certificate)
- Valid Photo ID Proof (Aadhar Card, PAN Card, Passport, Driving License, or Voter ID)
- Caste/Category Certificate (if applicable – SC/ST/OBC/EWS) issued by the competent authority
- Disability Certificate (if applicable, for PwD quota candidates)
